The Hospital to Home initiative is a community-based intervention for low-resource homeless individuals with complex health care needs who turn to hospital emergency departments for ongoing health concerns. The initiative engages these individuals to improve their health, stability, and quality of life. The purpose of the evaluation is to better understand who is served by Hospital to Home and the program’s impact on participants.
