For Adults
Targeted Case Management provides personalized, recovery-based support for adults living with a diagnosed serious and persistent mental illness. Person-centered case management helps individuals build independence, navigate services, and strengthen community connections. Monthly meetings support goals, coordinate care, and foster hope, resilience and progress toward recovery in everyday community life.
For Children
Targeted Case Management provides support for children diagnosed with significant mental health concerns and their families. Monthly meetings provide person-centered and whole-family help navigating mental health systems, building care and crisis plans, coordinating services, and strengthening family and community support so children can thrive in home, school, and community settings.
