More About Assertive Community Treatment

For Individuals
Consistent support to manage your severe and persistent mental illness and stay in your community. Our diverse care team is representative of the Hmong, Karen, Vietnamese, and Cambodian communities we serve. Receive help with medication, housing, employment, and recovery. We walk alongside you to reduce hospital stays and support long-term healing and independence.
For Families
When someone you love is living with serious mental illness, you deserve support too. We partner with families to reduce crisis, improve communication, and create stability. Our multilingual, multicultural team builds trust while helping you navigate care systems—so your loved one can thrive, and your family can feel more supported.

While ACT services are available to all Medicaid qualified adults, Wilder is under contract with Ramsey County to offer a sub-specialty ACT program with professionals experienced in meeting the unique needs of Southeast Asian communities.

Eligibility Requirements

Eligibility for ACT requires that individuals:

Have a significant difficulty with daily functioning due to their mental illness

Require continuous and high-intensity services to live in the community, and

Have a written statement from a licensed mental health professional recommending ACT level of care.
