This community health needs assessment was conducted for Southwest Health and Human Services to identify the health needs and assets of different cultural groups in Southwest Minnesota. Findings from this assessment will be used to inform the services provided by SWHHS and to help staff understand how to adjust or target their services to meet the health needs of these communities.
