This document includes seven data summaries that highlight information gathered during the CHNA process. They include demographic trends and social determinants of health; characteristics of patients served; early childhood and school-age youth health; and perspectives from the community, stakeholders, and clinicians on child health.
Wilder Research worked with Children’s Minnesota to complete their federally-required Community Health Needs Assessment (CHNA). The assessment process is intended to help hospitals understand and respond to the health concerns of the communities they serve.
