The Minnesota Department of Health was awarded a federal grant to implement a more coordinated system of services and supports for children and youth with special health needs and their families. Wilder Research is conducting an evaluation to measure the impact of the project on increasing stakeholder engagement and collaboration and families’ knowledge of the services and supports available to them.
