In-home support and care for adults living independently in senior and assisted living communities

Community Care Services provided by Wilder serve older adults and adults with disabilities, chronic health conditions, and/or memory loss who are living independently in their own homes within senior and assisted living communities. These services can be personalized and are perfect for adults who may need support in activities of daily living, personal care, or medication management in order to remain healthy and independent. And with in-home support, our Community Care Services also provide social companionship and respite time for family caregivers.

Companion Services

A variety of supports, including food prep, laundry, light housekeeping, trash removal, appointment accompaniment, and social companionship.

Personal Services

Supports offered include assistance with showering, changing clothes, basic grooming, medication administration and vital signs.

In-home Support

Ideal for clients transitioning from group respite. While clients are engaged in meaningful activities, including creative arts, music, games, exercise and more, caregivers are provided with a block of time away from home.

  • Ideal for clients transition from group respite to home respite.
  • Engaging activities for cognitive health and social wellbeing.
  • May include art, music, games, exercise and more.

Health Services

Staff Nurse provides initial assessment, on-going health monitoring, medication oversight and coordination of care with other health care providers.

  • Service plan development and monitoring

First step is to schedule an assessment for Community Care Services by getting in touch with us. 

We will work with you to get the support you desire on days and times that work best for you at your initial assessment. Daily, weekly, monthly - it's up to you! Wilder’s Community Care Services are affordable, flexible and can be personalized for you.

  • Initial assessment and service plan development: $200.00.
  • 30-minute visit minimum for any services.
  • After 30 minutes, visits will be charged at $9.00 for every 15 minutes.
