The purpose of this study was to create an evaluation plan by identifying outcome measures and data collection strategies.
The Living at Home Network includes 36 Living at Home and Block Nurse Programs in Minnesota that provide and coordinate community-based services to older adults. Wilder Research worked with the LAHN to develop an evaluation plan that will include collecting outcomes data and a framework for assessing costs and monetary benefits associated with the services.
