Funded by the Hennepin County Children’s Mental Health Collaborative, this effort is intended to enhance mental health screening practices in primary care settings, increase communication between primary care and mental health staff and, ultimately, increase access to mental health services. This report provides a summary of the efforts made by Partners in Pediatrics and Pediatric Consultation Specialists to provide co-located mental health services to youth during the past three years. More specifically, it describes the characteristics of youth served through this initiative, the types of referrals made following triage services, lessons learned, and plans for sustainability.
