This report describes results from an implementation and early outcomes evaluation of this service model established by the Minnesota Department of Human Services in 2016.

Behavioral Health Home service providers serve children and adults with serious mental illness and their families. BHH services aim to address the comprehensive physical, behavioral health, and social service needs of individuals in a holistic, coordinated manner.
