The Minnesota Department of Human Services Behavioral Health Home Services model serves children and adults with serious mental illness and their families with a multi-disciplinary team using a collaborative, person-centered, strength-based approach. BHH services aim to address the comprehensive physical, behavioral health, and social service needs of individuals in a holistic, coordinated manner.
