The SPICE-Bridge Partnership project aims to strengthen the connections and communication between community-based services and large health care systems, thus creating a model of improved care for older adults that enables them to continue living independently in their own homes for as long as possible. Evaluations examine the percentage of clients who remain in their homes and participate with the project, kept appointments with medical providers, surveys of hospital and clinic staff, and interviews with participants and direct service providers.
