Adult Case Management

Adult Case Management helps adults with serious and persistent mental illness monitor, coordinate and manage services necessary to meet  their complex mental health needs. Staff can help with access to community resources and needed medical, housing, social, educational, financial, vocational services. Ongoing evaluation ensures that services are appropriate to the level of care needed.

How to Access

Adult Case Management is available to adults 18 year or older living in Saint Paul and the east metro area. Referrals can come from individuals, families, or other community providers including Ramsey County Human Services. Services must be approved by Ramsey County and meet criteria for serious and persistent Mental Illness. Services are offered at Wilder Center and in home and community settings.
