Wilder Foundation Health and Wellness Programs strive to help older adults maintain control of their health and lives. Staff work to help older adults better manage chronic health conditions.

Living Well with Chronic Conditions: Chronic Disease Self-Management Program

Chronic Disease Self-Management is an evidence-based, six-week workshop series designed by Stanford University to help participants develop a toolbox of self-management skills to help them better cope with chronic conditions. Living Well with Chronic Conditions workshops are offered to people who are 60 years of age and older and who are living with one or more chronic conditions. Family and friend caregivers are also welcome to participate. Wilder and community partners serve the seven county metro area. How to Access

Wilder Foundation Health and Wellness Programs are available to seniors age 60 and older. Chronic Disease Self-Management workshops are held in various community settings, such as senior living facilities, community centers, libraries and clinics. A sliding fee scale is used to determine the fee for programs, but no one will be turned away for inability to pay.
