Community Services for Aging

Wilder’s concern for Saint Paul’s "older adults" dates back to 1906, when the Wilder Visiting Nurse Department became the Foundation’s first direct service program. It was designed to provide in-home nursing care to people who were ill, many of whom were elderly.

Today, Wilder’s Community Services for Aging serve a similar purpose, providing low-income adults and adults with disabilities living in the East Metro with innovative programs and services that allow them to live independent and healthy lives. Wilder’s Community Services for Aging operate several community-based programs for older adults and their families. For many older adults, remaining independent in their own homes is a key factor in how satisfied they are with their lives.

Wilder Community Center for Aging

Wilder’s newly renovated Community Center for Aging opened in January 2011. This Center positions Wilder to lead the charge for transforming systems of care by demonstrating innovative community-based solutions that work. Our goal is to provide expanded support for caregivers and cost-effective services that enable older adults to remain in the community – healthy, active and engaged.

At the Center, older adults and their caregivers can access a broad spectrum of care based on their individual needs. On any given day, an older adult might work on a project in the art room, visit a health care professional, and consult with a memory loss specialist with their caregiver – all under the same roof. This person-centered model enables older adults to remain in the community by using a holistic approach to care. Click here for tour information.

Caregiver Coaching

The Wilder Foundation, in collaboration with Eldercare Partners, offers a unique Caregiver Coaching program for those who care for an elderly or disabled family member or friend. The caregiver role can be overwhelming, but there is help (and hope) available. Wilder's coaching program provides education and support so caregivers can better manage the many challenges of this role, as well as connect with other available community resources. The ultimate goal is to help all involved continue to enjoy their lives together.

The Wilder Foundation has served Saint Paul's most vulnerable older adults for over 100 years. We create and implement innovative programs and services that offer opportunities for successful aging and collaborate with others to help meet the physical and emotional needs of older and disabled adults.

See how Wilder Caregiver Coaching supports the goal of helping older adults maintain independence and helps enhance the quality of life for them and the family members and friends who provide their care.
