Senior Outreach and Consulting

Knowing what kind of assistance is available or where to turn can make a big difference in the quality of life of older adults and their family caregivers.  Wilder’s team of licensed gerontological social workers provide care management and caregiver consultation services in partnership with Eldercare Partners, a collaboration of five Twin Cities nonprofit organizations.

Wilder Caregiver Services offer a personalized, convenient service to family caregivers to give them the knowledge, skills, and emotional tools they need to provide confident care. Wilder works with the caregiver in person, on the phone or via email to:
• Identify caregiving issues
• Offer education and training opportunities
• Manage caregiver challenges within a dynamic family system
• Use coaching techniques to seek balance for the caregiver role

Caregiver services are funded by a grant from the Metropolitan Area Agency on Aging (MAAA) with funds from the federal Older Americans Act.  Individual contributions are welcomed and help us reach more caregivers.

Wilder Geriatric Care Management and Consultation assists seniors and families with critical issues of aging including housing decisions, financial and legal issues, dementia and memory loss, and community resource planning.  Services include:
• Office consultations
• Comprehensive, multidimensional assessment
• A written report of recommendations defining options for current or future care
• Service planning, ongoing monitoring and coordination

The Care Management service is fee-based, and fee amounts depend on your service choices, including:
• Package 1:  Assessment and Recommendations
• Package 2:  Assessment, Recommendations, Service Planning and Coordination
• Hourly Services or Family Consultation
