Adult Day Health Services

Our Adult Day Health Services provide older adults with recreational and therapeutic activities, health monitoring, personal care/grooming assistance, nutritious meals and snacks, and social time.

Wilder Adult Day Health and Adult Day Health Memory Loss programs offer:

  • Independence – Our staff plan a variety of activities each day, designed to maximize physical and mental health and increase independence. A staff nurse conducts on-site health monitoring, such as blood pressure checks, blood sugar checks, and assistance with medications. Our social workers link participants to other community services so they can remain as independent as possible and avoid nursing home placement.

  • Convenience - Programs run Monday through Friday (and Saturday for Memory Loss), with early morning arrivals and late afternoon pick-ups. Transportation can be arranged for you.

  • Excellent care - Our staff is caring, knowledgeable and professional, and we pride ourselves on treating each client with respect and dignity. Staff members in our Memory Loss Program are specially trained to care for participants experiencing memory loss.

  • Family Caregiver Support - Our programs provide caregivers with support services, resources, and time for work, errands and needed rest.

The staff to client ratio at Wilder’s Adult Day Health programs exceeds state requirements.

Cost
Many funding sources are available: our sliding fee scale, insurance, and assistance payments. We can help you find the right solution.

General program
650 Marshall Ave., Saint Paul
Just South of I-94 and Dale, in our new location.
Program hours: Monday-Friday 7 a.m. - 5 p.m.

Memory Loss Program
753 East 7th Street, Saint Paul
Located in a secure space within the HealthEast Care Center in East Saint Paul, our Adult Day Health Memory Loss Program offers two care levels, one for early-stage memory loss and one for later-stage memory loss.
Program hours: Monday - Friday 6:30 a.m.-5:30 p.m., Saturday 9 a.m. - 3 p.m.
